Cancellation Terms: Month to Month Memberships Member may cancel this Agreement by providing 30 days notice as outlined above, except for cancellations due to relocation, disability or within 3 days. Cancellation must be received 30 days prior to the next billing cycle and requires 30 day written notice. If a member cancels on or after the 2nd of any month, then a payment will still be due the following month on the 1st. All new Month to Month Members assessed the $39 Rate Lock Guarantee at signup or be subject to 30-Days advance notice of a rate increase no more than (2) times within a 12-month period. Commit/Installment Memberships Member may cancel this Agreement due to relocation, disability or within 3 days. Cancellation must be received 30 days prior to the next billing cycle and requires 30 day written notice. If a member cancels on or after the 2nd of any month, then a payment will still be due the following month on the 1st. After your twelve (12) month contract, your membership automatically converts to a month-to-month membership at the applicable rates and terms thereof. Cancellation due to moving: · You must move your residence 25 miles or more from the club facility or any affiliated club. · You must provide sufficient proof that you are moving. · Please send in one of the following: · Lease agreement signed by both parties, and in member’s name. · New utility hook up or utility bill in member’s name. · Newly issued driver’s license. · Letter from your employer with copy of a current pay stub showing the new address. · Copy of closing papers on the purchase of a new home (in member’s name). · A one way plane ticket with a copy of a Visa or Green card. · If one of the above is not available, then the member must provide TWO of the following: · Current bank statement showing the new address. · Current major credit card statement showing the new address. · Post office change of address confirmation letter. · 30 day written notice of cancellation. You are still responsible for any payments that fall within the 30 day period from the date CrossFit Cedar Grove, LLC, receives the sufficient proof of move. · Payments will continue on the membership agreement until sufficient proof of move is received Cancellation due to Medical Condition: · Cancellation is allowed for a permanent or substantial disability. · You must provide documentation from your physician stating you have a permanent or substantial disability. · Must be on letterhead and signed by the physician. · You are still responsible for any payments that fall within the 30-day period from the date CrossFit Cedar Grove, LLC receives documentation from your physician. · Payments will continue until the proper documentation is received from your physician. I understand that CrossFit Cedar Grove, LLC has the right to send my account to collections and pursue legal action to recover any unpaid balances, processing fees, or other monies owed as per my signed contract.
By marking 'yes,' I understand that submitting this form does not automatically cancel my membership. I also understand that I am subject to the cancellation policy listed above as well as on my signed contract. I understand that I am responsible for all cancellation fees: 30 days notice on month to month memberships (1 month's fees and access for the 30 days) or the remaining contract balance for 12-month commit memberships paid in full at time of cancellation. I UNDERSTAND THAT I AM RESPONSIBLE FOR THE REMAINDER OF MY BALANCE IF I AM WITHIN CONTRACT.
By marking 'yes' I understand that CrossFit Cedar Grove, LLC has the right to send my account to collections and pursue legal action to recover any unpaid balances, processing fees, or other monies owed as per my signed contract.
By marking 'yes' I understand that if I am requesting a cancellation for medical reasons and I am within a contract/commit membership, I must email a doctor's note to [email protected] in order for my contract to be terminated early.
By marking 'yes' I understand that if I cancel and rejoin it will NOT be at my current rate but at the rates offered at the time at which I rejoin.
By marking 'yes' I understand that all cancellations require a minimum of 30 days advance notice from my next billing date even for moving or with a doctor's note. I understand that if I filled out dates that do not meet the 30-day notice criteria, my cancellation will be adjusted to the soonest dates available as per the requirements of my signed member agreement.
Requested Date of Cancellation - Please note: 30-days advanced notice of your next bill date is always required per your member agreement. If you select a date that does not meet this criteria, it will be updated to the soonest available date.

I agree that all holds without a doctor’s note and monthly membership hold and cancellations require 30 days' advanced notice prior to the next billing date. I understand that early termination of contract memberships will result in billing for the remaining term in full. I acknowledge that for paid in full memberships holds cannot be retroactive. I acknowledge that any chargebacks to avoid payment will lead to legal action, including a lawsuit in NJ Special Civil Court and a UCC-1 filing to recover any and all fees to enforce CrossFit Cedar Grove, LLC's member agreement.
