Payment Agreement: First payment is due 2 weeks from the date the contract is signed. Recurring payment schedule will begin 30 days from the date of the first payment. If MCL does not provide at least 30 leads in 2 consecutive months with an ad spend of $1000 the contract will be void. This clause is null and void if content is not provided by the doctor within 5-7 business days. I understand that this form is valid for a My Chiro Leads, LLC Membership. By signing this agreement, I am agreeing to have my card debited for the above-directed payment schedule. For security purposes, we will be taking card information over the phone. I also understand that for any declined payment I will be charged a $100.00 fee for additional processing. The document provided at the date of agreement serves as your statement, charges will appear on your monthly statement just like any purchase.
